1. Booking Conditions
1.1
All new patients are to pay the £80 consultation fee when booking their first appointment with Dr Nick Milojevic. The £80 consultation fee is to be paid over the telephone via debit or credit card. A receipt will be sent to you via e-mail along with the appointment confirmation; this is to confirm payment has been made and your appointment is confirmed.
1.2
Payments made via credit card will incur a 3% admin fee charge for all major credit cards and a 4% admin fee charge for American Express. All non UK based/foreign cards will also incur a 4% admin fee charge. Please note there is no admin fee charge for those using debit cards.

1.3
The £80 consultation fee will be deducted from the total cost of the treatment or future treatment. If the future treatment does not take place the £80 consultation fee does not become refundable. Future treatment is to be booked within three to six months of consultation. Please refer to all of section 2.
1.4
If the consultation/treatment appointment is cancelled the following charges will be applied:

1.5
If Dr Nick Milojevic cancels your appointment, a full refund will be provided.
1.6
Cancellation charges will not apply if consultation/treatment appointment is re-arranged to another day and time up to 3 days of the appointment taking place, appointment amendments will only be amended once free of charge.
2. Treatment Conditions
2.1
New patients may be asked to pay for treatment prior to the treatment taking place should treatment be opted for on the day of the consultation.
2.2
All patients are to complete a medical history form (please refer to section 2.3), read through the product information and sign the consent form as instructed prior and every time a treatment takes place. All patients will be sent the aftercare instructions via e-mail along with their appointment confirmation and given a copy on the day of their treatment.
2.3
All past patients will be asked to sign a declaration confirming that their medical history has not changed since the day of completion. It is the patient’s responsibility to inform Dr Milojevic of any changes to their medical history, however insignificant it may seem. Dr Milojevic reserves the right to ask for re-completion of a patient’s medical history at any time.
3. Payment Conditions
3.1
Full payment for consultation, treatment or products is required, credit will not be given.
3.2

4. Product Purchases
4.1
OBAGI products are available to order from Dr Nick Milojevic, following a consultation/discussion.; Please visit www.miloclinic.com for a full list of the products which are available to order. If you would like to place an order, please e-mail info@miloclinic.com listing the products you would like. Please see section 4.3.
4.2
If your treatment is the OBAGI 6-12 week course programme and you require further products, please contact info@miloclinic.com listing the products you require. You will be sent confirmation of the cost of each product once your request has been confirmed by Dr Milojevic. Once written confirmation from you has been received, full payment will be required and is to be paid via the telephone; please refer to section 3.2. You will be charged for postage and packaging, items will be sent recorded or special delivery. Please note until confirmation and payment has been received, the products will not be ordered. You will be sent a receipt via e-mail confirming payment has been made and your products have been ordered. Please note the products are non-refundable for hygiene reasons.
4.3
